The Certified Nurse Assistant (CNA) works cooperatively with all team members in planning and performing basic care. The Certified Nurse Assistant supports patient care delivery and is responsible for adhering to all standards of care and for promoting teamwork among coworkers.

Department Specific Competencies

  • Assists Rehab Therapists according to established guidelines with patient treatment including but not limited to the following activities: assists with Physical Therapy treatments such as bed mobility, sitting, transferring, standing, ambulating, as well as utilizing/monitoring equipment appropriately, using tilt table assists with Occupational Therapy treatments including bathing/showering/ADL/feeding, transferring and others as directed prepares patient to be available for therapies at scheduled time transports patient to rehab gym/solarium for meals/ speech/other treatments as directed

  • Implements the patient care plan by providing direct patient care for those patients assigned including but not limited to the following activities: gives baths, oral hygiene, skin care, backrubs assists patients to and from bathroom positions patients and changes position appropriately according to care plan instructions ensures patients receive prescribed diet, assists with feedings as needed, places tray conveniently collects specimens as instructed using appropriate technique takes vital signs and notifies RN of any change or abnormal finding assists/performs treatments as directed by RN/LVN including: foley care, application of binders, FSBS, sitz baths, and post mortem care, others as directed.

  • Reports any changes in patient condition to RN/LVN; receives report.

  • Documents accurately in the patient record all activities. Documents accurately on I and O sheets all intake and output for assigned shift, including gomco or wall suction, hemovac, T-tubes and foley outputs documents care provided and patient responses according to policy and procedure.

  • Orients/instructs patient/family regarding the physical environment, routines and general rules: call system, bed controls, telephone, television, visitor policy, meal times, bathroom/shower, smoking policy, roommate introduction
