Position Summary Â
The Palliative Care RN is responsible for modeling the Providence at Home with Compassus values of Compassion, Integrity, Excellence, Teamwork, and Innovation, and for promoting the Providence at Home with Compassus philosophy using the 6 Pillars of Success as the foundation. S/he is responsible for upholding the Code of Ethical Conduct and for promoting positive working relationships within the company, among all departments, and all external stakeholders. Â
Under the supervision of the Director of Clinical Services, the Palliative Care Registered Nurse provides services to palliative care patients and their families following nursing processes and in accordance with the Nurse Practice Act. The Nurse participates as a member of the interdisciplinary team, providing clinical support, communication, and coordination of care across settings. Â
The Palliative Care Nurse functions as an IDT member to provide telephonic and telehealth routine and emergency assessment, educational, and evaluative services to meet the needs of patients and their families. S/he assists the IDT in the assessment and management of the patient's prognosis and symptom burden, and helps maintain their maximum level of comfort and coping. The RN will also participate in evaluating palliative care patients for transition to hospice programs.  Â

The employee must be competent in interpreting a patient’s self-report or behavior, considering cognitive, physical, emotional/psychosocial, and developmental factors. Treatment should be individualized to reflect the patient’s specific needs. Â

Position Specific Responsibilities Â
Utilizes the nursing process to assess patient status, collaborate with attending staff to intervene appropriately, and evaluate patient response. Â
Participates as a member of the interdisciplinary team addressing patient and family needs. Â
Observes, evaluates, and contributes information regarding psychosocial, spiritual, physical, and financial conditions affecting the patient and family. Â
Serves as an advocate for the patient/family unit. Â
Develops and maintains contact with appropriate community agencies and services to promote inter-agency cooperation and facilitate referrals. Â
Facilitates and supports decision-making around end-of-life planning, including discussions about treatment options, advance directives, and communication of wishes with family and friends. Â
Maintains patient records in accordance with state, federal, and organizational requirements. Â
Engages in professional development related to position responsibilities. Â
Participates in palliative care committees and task forces. Â
Performs other duties as assigned. Â

Required: Associate’s Degree in Nursing Â
Preferred: Bachelor’s Degree in Nursing Â

Physical Demands and Work Environment:Â The demands of this role necessitate a team member to effectively perform essential functions. Adaptations can be made to accommodate team members with disabilities. Regular standing, walking, and manual dexterity are fundamental, along with the ability to lift and move objects up to 50 pounds. Visual acuity requirements include close and distance vision, color and peripheral vision, depth perception, and the ability to adjust focus. In a healthcare setting, exposure to bodily fluids, infectious diseases, and conditions typical to the field is expected. Routine use of standard medical equipment and tools associated with clinical care is essential. Compensation range:Â $50.18 - $77.91Â Â

The amounts listed are the base pay range; additional compensation may be available for this role, such as shift differentials, standby/on-call, overtime, premiums, extra shift incentives, or bonus opportunities Â
