We are seeking a compassionate and motivated Nurse Practitioner (NP/CRNP) or Physician Assistant (PA) to join our growing practice in Towanda, Pennsylvania.
Our providers deliver comprehensive medical care to residents in Skilled Nursing, Assisted Living, and Independent Living communities. We offer a collaborative, provider-focused practice model built around clinical autonomy, manageable patient volumes, strong support, competitive compensation, and work-life balance. As a Nurse Practitioner or Physician Assistant, you will provide primary and chronic care management services to residents in long-term care communities.

Manageable Patient Volume- Our providers typically see approximately 20 patient visits per day, allowing time for thorough assessments, meaningful patient interactions, and high-quality clinical decision-making.

Each provider receives support from a dedicated Regional Nurse Coordinator who assists with:
- Daily scheduling and care coordination
- Documentation and note management
- Facility communication
- Prescription refills and medication requests
- Forms, orders, and administrative tasks
- Communication with nursing staff, families, pharmacies, and other healthcare partners
Compensation
- Up to $125,000 annually with Total Compensation Package: $133,000–$150,000+ annually

- Experience in geriatrics, long-term care, skilled nursing, family medicine, internal medicine, or primary care is preferred but not required.
