Sage Group HealthCare Staffing is seeking Travel LPN LTC for 13-week travel / contract position in Traverse City, MI. Accepting both local and traveler candidates
Position Details:
Position Name: Travel LVN/LPN Long Term Care

Contract Length: 13 weeks
Pay Rate: $1600 to $1720 Weekly Gross pay based on a 40 hour work week (Includes non-taxable travel/housing and M&I stipend available for qualified/eligible travelers)
Shifts available: Evenings
Schedule: 40 hours per week guaranteed, 5 X 8-hr shifts per week with Every other weekend, holiday rotation required if scheduled. No blocked scheduled
Position Requirements:
- At least two years of recent/current SNF/LTC or Med Surg: Recent/Current - Required
- At least 1 travel contract completed - Required
- Covid Vaccination Proof or exemptions - Required
Education, License and Certification Requirements:
- LPN License for State of Michigan - Required
- BLS Certification from AHA - Required
Job Description:
- Prescribes and adheres to facilities protocols and kindness in all personal and professional interactions while on duty in and around the facility.
- LPN demonstrates great customer service by embodying facilities three core principles of Exceptional Care, Passionate Services and Unexpected Luxury in all of their job duties and responsibilities.
- Under the supervision of RN, LPN assumes responsibility and accountability for a group of residents/patients for a shift of duty.
- Nursing care is provided through assessment, implementations, and evaluation of the plan of care.
- LPN adheres to the standards of care for the area, manages the environment to maintain resident/patient safety, and supervises the resident/patient care activity performance by nursing assistants and helpers.
- Follows all facilities policies and procedures.
- Performs duties as defined by the State Nurse Practice Act. Follows where applicable JCAHO standards.
