What is PACE North? PACE stands for Program of All Inclusive Care for the Elderly. We are a unique option for individuals 55 and older who qualify for nursing facility level of care and wish to remain in their homes as they age. What does that mean? We care for the elderly at our day center on Garfield near Hammond during regular business hours and as needed in their home. We began serving the elderly in 2019 and our service model is so popular it’s growing quickly!

What does a Physical Therapist at PACE North do? Physical Therapists at PACE North are a key component in helping our participants in the healing process. Since PACE North is the insurance company for our participants, there will be no productivity. Care is determined by the Interdisciplinary Team. They provide coordination of care and provide training to family members, and PACE North staff. You will be going to our participants homes and providing home care as well as providing care at our center, With lots of one on one care for our participants. Will help in the discharge planning for our participants from the hospital, assisted living, or skilled nursing facilities. 

Requirements

What qualifications should you bring to the table? We're looking for that special person who loves helping others with a positive approach. We'd appreciate a minimum of 1 year experience working with the frail or elderly. You will also need a Masters or a Doctor of Physical Therapy degree, be CPR certified, and a valid Mi driver's license.
