Position Overview

The Registered Nurse (RN) Case Manager provides comprehensive assessments and care coordination for high-risk members in a home health setting. The RN collaborates with providers and interdisciplinary teams to create individualized care plans, ensuring appropriate levels of care and optimal patient outcomes.


Key Responsibilities

  • Conduct comprehensive patient assessments and develop individualized care plans.

  • Collaborate with physicians, social workers, and other healthcare professionals to coordinate patient care.

  • Facilitate care transitions and ensure continuity of care post-discharge.

  • Monitor progress and adjust care plans as needed for optimal outcomes.

  • Identify barriers to care and implement non-medical supports (housing, transportation, etc.).

  • Maintain accurate documentation of clinical and financial outcomes.

  • Promote preventive care and wellness education for patients and families.

  • Advocate for patient rights, privacy, and confidentiality.


Qualifications

  • Associate’s Degree in Nursing (BSN preferred)

  • Active, unrestricted Registered Nurse license 

  • Strong ability to interpret complex clinical data and make informed decisions

  • Prior Case Management experience preferred

  • Homecare Homebase and OASIS proficiency required
