Small Joys provides integrated behavioral health services by partnering with primary and specialty care physicians to support patient wellbeing and improve health outcomes. We are currently hiring a Behavioral Health Integration Manager for a full-time in-person role supporting multiple partner care facilities.

Responsibilities

  • Conduct patient intake assessments and assist in developing individualized treatment plans

  • Provide follow-up sessions and care coordination for patients enrolled in behavioral health programs

  • Document clinical notes using standardized templates and maintain accurate patient records

  • Collaborate with psychiatrists and primary care physicians regarding care plans and patient progress

  • Act as a liaison between the behavioral health care team and primary care providers

  • Manage patient registry and track time spent on each patient for billing and reporting

  • Coordinate with patients’ family members when necessary to gather medical and behavioral history

  • Support behavioral health service delivery across assigned partner facilities



Requirements

Qualifications

  • Master’s degree in Clinical Psychology, Counseling, or a related field

  • Master of Social Work (MSW) is also considered

  • Minimum 2 years of relevant behavioral health experience

  • Strong documentation, coordination, and communication skills

  • Ability to work collaboratively with multidisciplinary healthcare teams

One individual will support these locations as part of the weekly schedule.
