Community Based Clinical & Care Management Services

Salary range:

$48K-58K 

40 hours per week; Troy location

Benefits:

Job Description:

Under the direction of the Senior Clinical Supervisor, the therapist will provide individual, family and group therapy to assigned clients in home, office, telehealth and community settings.

Responsibilities:

Develop and maintain comprehensive clinical assessments and treatment plans on assigned individuals and families utilizing St. Anne’s Electronic Health Record

Provide therapeutic services to clients in schools, community and in home as needed

Complete discharge summary and assist with discharge planning on each assigned case

Provide aftercare services for assigned cases post-discharge

Keep informed of current trends in clinical, trauma-informed and evidence-based practices

Adhere to NASW, AMHCA, NYS Justice Center, St. Anne and ATSA policies and guidelines

Attend all clinical department staff and supervisory meetings as required including in person, once weekly with supervisor at the Hudson Falls Office


Requirements and Education:

MSW/ LMSW/LCSW, MCH, LMHC or other relevant degree required; experience working with individuals, families and groups preferred; recent Masters level graduates encouraged to apply; ability to function as part of a multidisciplinary team; valid driver’s license required.
