About the Role

Unity House’s Transitional Apartment Services Program (TAS) is seeking a compassionate and motivated Case Manager to join our team! This position supports individuals living with mental illness on their journey toward independence and community integration.

 Key Responsibilities

  • Provide supportive services to program participants with mental illness, ensuring compassion, dignity, and respect.

  • Assist participants in developing daily living skills and achieving personal goals.

  • Maintain documentation and participant charts in compliance with Office of Mental Health (OMH) standards.

  • Communicate regularly with healthcare and social service providers to coordinate care.

  • Assist with light to moderate cleaning in participant apartments as needed.

  • Promote an environment of inclusion, belonging, and empowerment for all participants.

Qualifications: 

  •  Education: High School Diploma or equivalent required.

  • Experience: Minimum of one (1) year of qualifying experience in human services; experience working with diverse populations strongly preferred.

  • Licenses: Must have reliable transportation and a valid NYS driver’s license.

  • Language Skills: Bilingual candidates are strongly encouraged to apply.
