Join the Unity House of Troy YAMICA Program as a Case Manager and support young adults throughout Rensselaer County in achieving greater independence, stability, and personal success. This position offers the opportunity to build meaningful relationships while helping individuals access resources, strengthen daily living skills, and work toward educational and employment goals in a supportive, community-based environment.

Responsibilities

  • Engage in ongoing, consistent interactions with clients to build trust and support progress toward individualized goals. 
  • Provide or assist with crisis prevention and intervention when needed. 
  • Assist clients in identifying and pursuing educational and employment opportunities. 
  • Provide transportation to appointments and community services as needed. 
  • Make referrals and connect clients with appropriate supportive services and resources. 
  • Advocate on behalf of clients with community agencies and service providers. 
  • Conduct home visits and field-based services as needed. 
  • Teach and reinforce daily living and independent living skills. 
  • Support symptom management and overall wellness. 
  • Encourage healthy social development and community integration. 
  • Promote an environment centered on inclusion, dignity, and belonging. 

Qualifications

  • Bachelor’s or Associate’s degree in health or human services with one year of experience working with diverse populations preferred. 
  • High school diploma or GED with at least two years of relevant experience required. 
  • Valid, clean New York State driver’s license required. 
  • Must have reliable transportation available for daily work responsibilities. 
  • Bilingual candidates are encouraged to apply.
