The Choice Hospice Nurse Practitioner is a passionate provider and hospice advocate- a proven clinician responsible for helping shepherd the care of the seriously ill. The ideal candidate will have a personality type predisposed to thrive in an unstructured environment and be comfortable responding to patient needs while being a supportive team member with an excellent ability to communicate with team members.Â
As a Hospice Nurse Practitioner, you will:
Collaborates with the IDG in assuring appropriate evaluation and palliative treatment of terminal disease.
Consults with and educates the patient/family regarding disease process, self-care techniques, end-of-life care. Provides training to other staff as needed.
Perform Face-to-Face visits at the patient's place of residence.
Interacts with attending and consulting physicians as necessary regarding palliative care, in collaboration with hospice physicians.Â
Provides clinical assessment results to hospice physician after making face-to-face encounters with patients, as assigned.
Keeps up with and maintains education/knowledge in pharmacology and palliative medicine.Â
Knowledgeable of hospice criteria.
Requirements
Current STATE RN and Advanced Practice Nurse License
Valid Driver’s License
Valid Auto Insurance
AHA CPR Certification
HomeCare HomeBase experience preferred
