Nurse Practitioner PRN

The Choice Hospice Nurse Practitioner is a passionate provider and hospice advocate- a proven clinician responsible for helping shepherd the care of the seriously ill. The ideal candidate will have a personality type predisposed to thrive in an unstructured environment and be comfortable responding to patient needs while being a supportive team member with an excellent ability to communicate with team members. 

As a Hospice Nurse Practitioner, you will:

• Collaborates with the IDG in assuring appropriate evaluation and palliative treatment of terminal disease.

• Consults with and educates the patient/family regarding disease process, self-care techniques, end-of-life care. Provides training to other staff as needed.

• Perform Face-to-Face visits at the patient's place of residence.

• Interacts with attending and consulting physicians as necessary regarding palliative care, in collaboration with hospice physicians. 

• Provides clinical assessment results to hospice physician after making face-to-face encounters with patients, as assigned.

• Keeps up with and maintains education/knowledge in pharmacology and palliative medicine. 

• Knowledgeable of hospice criteria.

Requirements

• Current STATE RN and Advanced Practice Nurse License

• Valid Driver’s License

• Valid Auto Insurance

• AHA CPR Certification

• HomeCare HomeBase experience preferred
