Job Title: Long-Term and Post-Acute Care Nurse Practitioner

WHAT YOU WILL DO AS A LONG-TERM AND POST-ACUTE CARE NURSE PRACTITIONER:

Provide direct care for patients with chronic and acute conditions
Collaborate with physicians to guide treatment plans
Partner with facility leadership and interdisciplinary teams
Help support MedCap Health's rollout into a new community

Qualifications:

1+ years of experience in post-acute/SNF/LTC
Board-certified geriatric, family, or adult Nurse Practitioner (ANCC, ACHPN, or AANP)
Post-acute, long-term, and/or skilled nursing facility experience

YOUR SCHEDULE

Enjoy the consistency of day shifts Monday through Friday with no nights, giving you the balance you need while working in a meaningful and growing role.

WHAT WE OFFER OUR TEAM:

Pay: This contract role offers a highly competitive annual range of $150,000 to $200,000, rewarding your expertise and impact.
