Description

Appalachian Mountain Health, a federally qualified health center (FQHC), is seeking a compassionate and mission-driven Physical Therapist to provide high-quality rehabilitative care to patients across the lifespan. The Physical Therapist will evaluate, diagnose, and treat patients with physical impairments, functional limitations, injuries, and chronic conditions in a community health setting.

The ideal candidate is committed to improving health outcomes for underserved and diverse populations through evidence-based care, patient education, and interdisciplinary collaboration.

Requirements

Essential Duties and Responsibilities

  • Conduct comprehensive physical therapy evaluations and develop individualized treatment plans.
  • Provide therapeutic interventions including exercise therapy, manual therapy, neuromuscular re-education, gait training, balance training, and pain management techniques.
  • Monitor patient progress and modify treatment plans as clinically appropriate.
  • Educate patients and caregivers regarding treatment plans, home exercise programs, injury prevention, wellness, and self-management strategies.
  • Document evaluations, progress notes, treatment plans, and discharge summaries in the electronic health record (EHR) in a timely and accurate manner.
  • Collaborate with primary care providers, behavioral health clinicians, care coordinators, and other interdisciplinary team members to support whole-person care.
  • Participate in quality improvement initiatives, care coordination activities, and population health programs.
  • Ensure compliance with all federal, state, payer, and FQHC regulatory requirements.
  • Maintain patient confidentiality in accordance with HIPAA standards.
  • Support organizational goals related to access, quality, patient satisfaction, and health equity.
  • Participate in community outreach, patient education events, or wellness programs as needed.


Required Qualifications

  • Doctor of Physical Therapy (DPT) degree from an accredited physical therapy program.
  • Current and unrestricted Physical Therapist license in the state of [Insert State].
  • Current CPR/BLS certification.
  • Strong clinical assessment and treatment skills.
  • Experience documenting in an electronic health record system.
  • Excellent communication and interpersonal skills.
  • Commitment to serving culturally diverse and underserved populations.

Preferred Qualifications

  • Minimum of one (1) year of outpatient or community health experience.
  • Experience working in an FQHC, community health center, or integrated care environment.

  • Familiarity with value-based care and population health concepts preferred.


Physical Demands and Work Environment

  • Ability to stand, walk, bend, lift, and assist patients throughout the workday.
