About the Role:

We are seeking a highly skilled and dedicated MDS LPN or RN to join our team at BEL Oak of Union. As an MDS, you will be responsible for providing exceptional care to our patients and ensuring that their needs are met. You will work closely with our team of healthcare professionals to develop and implement care plans that are tailored to each patient's unique needs. Minimum Qualifications:

  • Active LPN or RN license in the state of Missouri
  • Minimum of 2 years of experience in a long-term care setting (preferred)

  • Excellent organizational and time management skills

Preferred Qualifications:

  • Experience with MDS assessments and care planning
  • Experience with electronic medical records systems

Responsibilities:

  • Conducting patient assessments and developing individualized care plans
  • Administering medications and treatments as prescribed by physicians
  • Monitoring and documenting patient progress and communicating with healthcare team members
  • Providing education and support to patients and their families
