About the opportunity:
 
The Nurse Navigator role is intended to guide the entire life-cycle of patient care within the department. The Navigator is the liaison between all components of care; providing information seamlessly to the patient/ family, and between care providers in every care setting the patient uses.
 
What you will do:
  • Consult with patients by phone or in person to determine specific needs, determine referrals to care.
  • Build a patient-family centered relationship.
  • Provide initial admission assessment, screenings, and assess for barriers to care.
  • Report to team.
  • Attend and/or review provider appointments; reinforce education and information provided by caregivers.
  • Develop and deliver a Care Plan with treatment summary to patients and/or sign consents and declinations and reinforce education on treatments accepted or declined.
  • Organize evidence-based care plan and present to patients for discussion.
  • Follow up as required with patient and providers.
  • Sustain a patient centered relationship with the patient and family throughout the course of care and into post-care as needed.

What you will need:
 
Experience:
  • 2 years of experience working directly with patients and providers required.
  • 2 years of experience in specialty associated with application preferred.
Education:
  • Minimum of Associate Nursing Degree for positions hired after 07/01/2018.
License(s) and Certification(s):
  • Licensed as a Registered Nurse in the state of Colorado or from a valid compact state required
  • Basic Life Support (BLS) required   
    • Department specific:
      • If Oncology - Oncology Certified Nurse (OCN) preferred.
      • If Obstetrics - Registered Nurse Certified – Inpatient Obstetric Nursing (RNC-OB) required

Pay is based upon relevant education and experience per hour.
