What you'll do

  • Complete 6–9 in-home health assessments/day
  • Review medical history and risk factors
  • Collect vitals, labs, and screening data
  • Document findings in the EMR
  • Educate members on next steps
  • Coordinate follow-up if red flags are identified
  • Travel throughout Clark County, WA
