Sage Group HealthCare Staffing is seeking an Outpatient LPN or a travel/contract position in Vandalia, IL

Pay Rate: $1728 Weekly Gross Pay based on a 40-hour work week (Includes Non-taxable travel/housing and M&I stipend available for qualified/eligible travelers)
Shifts available: Days
Shift timings and Schedule: 12-hr, Days, 7.00a - 7.30p, 36 hours per week, 3 X 12-hr shifts per week - Weekend on holiday rotation as needed
Position Requirements:
- Current LPN experience: 2+ years - Required
- Outpatient (Doctors office) experience - Highly preferred
- Prior travel experience: Highly preferred
Education, License and Certification Requirements:
- LPN License for State of Illinois - Required
- BLS Certification from AHA - Required
Job Description:
- Answers phone calls from patients, hospitals, extended care facilities, pharmacies, and physician offices and directs the information to the appropriate provider as needed.
- Assists providers with patient exams/ treatments/ procedures.
- Assists the providers by performing patient calls regarding test results and follow-up care as instructed by the provider.
- Collects pertinent data and information relative to the patient's health or the situation. Recognizes normal and abnormal findings when gathering data.
- Coordinates follow-up care, including scheduling of tests and referrals as ordered by the physician in a timely and accurate manner.
- Documents appropriately and concisely the assessment, care provided, and data collection in the medical record.
- Ensures that pre-certification of tests/ consults is obtained.
- Performs and documents laboratory duties which may include phlebotomy, specimen collection, and performance of routine basic waived testing.
- Submits prescriptions and refills to the pharmacy ordered by the physician in a timely and accurate manner.
- Teaches patients and family members about the plan of care and documents education.
