Physical Therapist – Home Health
Join Choice Health at Home as a Physical Therapist and help patients maintain, improve, and restore their mobility and physical function in the comfort of their homes. In this role, you will develop individualized treatment plans, evaluate patient progress, and collaborate with families and healthcare professionals to deliver high-quality, compassionate care.
Key Responsibilities
- Perform and document comprehensive initial examinations.
- Evaluate clinical data to identify patient needs and establish a physical therapy diagnosis before intervention.
- Develop and implement individualized treatment programs designed to improve or restore physical function, alleviate pain, and prevent further dysfunction.
- Evaluate treatment effectiveness at appropriate stages and adjust interventions to maximize patient outcomes.
- Apply infection-control practices and universal precautions during patient care.
- Ensure continuity and quality of care through accurate, timely documentation.
- Communicate effectively with patients, families, team members, physicians, and other healthcare professionals.
- Follow the agency’s philosophy, administrative policies, and clinical standards.
- Complete all required documentation within company guidelines.
Qualifications
- Successful completion of an accredited physical therapy program.
- Current, valid Physical Therapist license.
- Successful completion of the applicable state licensure examination.
- Current CPR certification.
- Valid driver’s license and automobile insurance.
- At least one year of home health experience preferred.
- Homecare Homebase experience preferred.
Physical Requirements
- Ability to stand, walk, sit, climb, crawl, squat, and kneel for extended periods.
- Ability to lift, carry, pull, or push up to 100 pounds.
