The VNA is seeking a Hospice Scheduler to support staff and patients regarding the scheduling and triage of patients, with additional administrative and office duties to support the agency, department, immediate supervisor, their direct reports and staff. Perform duties under supervision of the Director of Home Health or designee, such as Patient Care Manager.

Responsibilities:

  • Demonstrates knowledge of and the ability to perform scheduling duties with minimum supervision.
  • Under the direction and support of the clinical manager triages calls from staff patients. families and physician offices
  • Communicate with contracted pharmacy and DME as needed for coordination of patient care
  • Assists in taking hospice referrals, enters pertinent demographic information in HCHB and schedules admissions as back-up to Hospice Coordinator.
  • Participates as part of the clinical support team, providing information, directions with an internal customer service focus.
  • Completes courtesy phone calls to notify physician offices when patients expire.
  • Prepares monthly HHA and Nursing calendar and daily schedule.
  • Tracks patient recertifications to ensure compliance with Medicare CoPs.
  • Provides RN case managers with updated medication profiles and care plans for patients in homes and facilities. Assures Interdisciplinary Plan of Care form is provided to all patients in nursing facilities.
  • Collects and distributes pharmacy cards and medication reviews to RN case managers.
  • Completes all scheduling tasks in HCHB (visit verification, missed visits.
  • Runs and reviews HCHB scheduling reports daily, weekly, and monthly to ensure scheduling process is effective, efficient, and compliant.
  • Demonstrates analytical skills when maintaining schedules/staffing to ensure appropriate staffing ratios quality and continuity.
  • Collaborates with all disciplines to ensure accurate scheduling, continuity, and current patient information.
  • Assigns care teams to new patients based on geographic location and caseload.
  • Anticipates staffing needs and notifies PCMs when per diem staffing is indicated.
  • Maintains vacation, weekend and holiday on-call schedules.
  • Assures compliance with Medicare CoPs with regard to visit frequency.
  • Prepares reports on efficiency, time studies and productivity as requested.
  • Perform patient visits as needed and requested by PCM
  • Performs clerical tasks for teams.
  • Provides administrative office support/coverage as requested

Qualifications:

  • LPN licence required
  • Hospice home care experience required
  • Six months to one year related experience and/or training
