The VNA is seeking a full-time Physical Therapist for our Indian River County Home Health division. Under the order of the physician, the Physical Therapist assumes primary responsibility for providing skilled physical therapy and health teaching to patients and their significant others/caregivers in the home setting.

Responsibilities:

1. Conduct physical assessment/histories to determine health problems, homebound status and follow plan of care/treatment plan in collaboration with the physician.

2. Observe patient's condition to monitor response to treatment and communicate/document changes to physician.

3. Counsel and teach home program to patient and significant other /caregiver to accomplish goals.

4. Formulate appropriate discharge plans.

5. Collaborate with scheduler on a regular basis for coordination of patient care/schedules.

6. Maintain clinical EMR and paper records according to agency, state and federal guidelines.

7.  Develop home health aide assignments and provide supervision/guidance on physical therapy only cases.

8. Attend case conferences/team meetings as scheduled.

9. Attend skills-fair and other mandatory educational programs.

10. Provide supervision to licensed physical therapist assistants.

QUALIFICATIONS  To perform this job successfully, an individual must:

• Possess current PT license in the State of Florida
• Have a minimum of 1year experience as a PT
