Registered Nurse Home Health Care - Full-Time

Established with a commitment to all-encompassing, Medicare-certified care, Indian River Home Care is a concierge-style home health care agency seeking a full-time registered nurse to support our patients in Indian River County.

As a full-time registered nurse for Indian River Home Care you are given matchless support to determine the corporal needs of our patients based on your expertise, while our motivated and dedicated philosophy empowers you to work autonomously. We are a close-knit team of clinicians who provide services according to our patients' needs and goals.

This full-time position includes a one weekend per month commitment.

Your Qualifications should include:

Job Type: Full-time

Responsibilities:

  • Provide skilled nursing care to patients in their homes, including medication administration, wound management, and vital signs monitoring.
  • Conduct thorough patient assessments and develop individualized care plans aligned with physician orders and patient needs.
  • Educate patients and their families on disease management, medication usage, and health maintenance strategies.
  • Coordinate with physicians, therapists, social workers, and other healthcare professionals to ensure seamless care delivery.
  • Document all patient interactions accurately using electronic health record (EHR) systems such as Epic or Cerner while adhering to HIPAA regulations.
  • Perform specialized procedures such as IV insertion, catheter care, ostomy management, and sterile dressing changes following aseptic techniques.
  • Monitor for signs of acute pain or deterioration and respond promptly with appropriate interventions or triage protocols.

Requirements

  • Valid RN license in the state of practice with current CPR certification (Basic Life Support).
  • Proven experience in home health care or related settings such as hospitals, nursing homes, or assisted living facilities.
  • Strong clinical skills including wound care (sutures), IV infusion therapy, medication administration (including injections), and infection control practices.
  • Familiarity with EMR/EHR systems like Epic or Cerner; experience with ICD coding (ICD-10/ICD-9) is a plus.
  • Knowledge of diverse patient populations including geriatrics, pediatrics, individuals with developmental disabilities or dementia care needs.
  • Critical care experience such as ICU or emergency room background is highly desirable; experience in trauma centers or post-acute care settings is beneficial.
  • Ability to perform comprehensive physical examinations and diagnostic evaluations; proficiency in laboratory specimen collection and processing preferred.
