Responsibilities

Evaluate residents and develop individualized treatment plans
Provide skilled physical therapy for orthopedic, neurologic, and mobility conditions
Monitor patient progress and adjust care plans as needed
Collaborate with interdisciplinary care teams
Maintain accurate and timely clinical documentation
Participate in discharge planning and continuity of care

Qualifications

Graduate of an accredited Physical Therapy program
Active, unrestricted Florida Physical Therapist license
Skilled nursing or rehabilitation experience preferred
Strong communication and organizational skills
