Join Our Therapy Team at Visalia Post Acute

Visalia Post Acute is seeking a compassionate and skilled Occupational Therapist (OT) to join our dedicated rehabilitation team. This full-time opportunity is ideal for an OT who is passionate about helping residents achieve maximum independence, function, and quality of life in a skilled nursing and rehabilitation setting.

General Purpose

The Occupational Therapist (OT) evaluates and treats residents, develops and implements individualized treatment plans, communicates with physicians, families, and interdisciplinary team members, and maintains accurate and timely documentation in the medical record. The OT also provides clinical guidance and education to staff to support optimal resident outcomes.

Essential Duties and Responsibilities

  • Evaluate residents within 24 hours (or one business day) of physician referral; document delays appropriately

  • Develop individualized treatment plans and obtain physician approval for services

  • Provide occupational therapy treatment per the physician-approved plan of care

  • Supervise Certified Occupational Therapy Assistants (COTAs), aides, and students in accordance with the state practice act

  • Communicate resident progress, challenges, and plans with the rehab and interdisciplinary teams

  • Participate in patient care conferences, utilization review meetings, and rehab meetings as needed

  • Complete evaluations, daily treatment notes, weekly progress notes, recertifications, and discharge documentation per regulatory and payer requirements

  • Educate residents, families, and caregivers on safety, adaptive techniques, and home programs

  • Participate in discharge planning and coordination of care

  • Recommend and facilitate ordering of durable medical equipment (DME) to promote functional independence

  • Conduct in-services and training for facility staff as assigned

  • Report equipment issues promptly to ensure safe and effective operation

  • Maintain professional, respectful relationships with residents, families, and staff

Supervisory Responsibilities

  • Supervise Certified Occupational Therapy Assistants (COTAs), aides, and students

Qualifications

Education & Experience

  • Bachelor’s degree in Occupational Therapy required

  • Master’s or Doctorate degree in Occupational Therapy preferred

  • Skilled nursing or geriatric experience preferred but not required

Licensure & Certifications

  • Current, active California Occupational Therapist license in good standing

  • Must meet all continuing education requirements per state regulations

Skills

  • Proficient in electronic documentation systems (Casamba and PointClickCare preferred)

  • Strong clinical judgment, communication, and organizational skills

  • Ability to work independently and as part of a collaborative team

Physical Requirements

  • Frequent standing, walking, pushing, pulling, and reaching

  • Ability to lift and move up to 50 pounds frequently

  • Occasional stooping, kneeling, crouching, and balancing

  • Visual and auditory acuity sufficient for patient care and documentation

  • Reasonable accommodations may be made to enable individuals with disabilities to perform essential functions

Work Environment

  • Skilled nursing and short-term rehabilitation setting

  • Noise level typically low to moderate
