Summary Job Description
The Case Planner in the TFBH program provides casework, group work, and advocacy services to children and families. The Case Planner focuses on enabling optimal bio-psychosocial development of the child by supportive work with foster parents and rehabilitative work with the child鈥檚 family.
Performs both field and office work in handling a caseload for the agency. Work involves gathering information and providing services to meet child鈥檚 needs, focusing on permanency planning; dealing with governmental and community agencies; counseling individuals and transmitting information on cases through completion of agency and governmental documents. Work is reviewed through conferences with a Casework Supervisor.

Principle Responsibilities
路 Obtains data regarding child鈥檚 and family鈥檚 social and medical history. Provides case planning.
路 Advocates for and obtains appropriate services for the child, including educational, clinical, and recreational services.
路 Meets regularly with foster parents to clarify issues, provide and/or obtain concrete and supportive services. Monitors the quality of care provided to the child. Keeps foster parents advised of child鈥檚 condition and prognosis.
路 Engages the child鈥檚 family in casework and group work activities aimed at sustaining relationships through visiting. If possible, works towards family reunification. Assist with adoption related activities if necessary.
路 Arranges for regular training of foster parents.
路 Share 24-hour 7-day emergency cell phone coverage with the Behavior Specialist. Responds to emergencies after hours or on weekends.
路 Performs related duties as required.
Required Qualifications and Experience
- MSW; M.A. + 1 year experience; B.A. + 3 years related experience; or B.A. + 5 years unrelated experience.
Travel Requirements
This position offers a full-time, 35 hours per week, hybrid/in field schedule in Wading River, NY. It requires routine travel throughout Suffolk and Nassau County.
