Field Nurse Practitioner to provide compassionate, high-quality care to patients across various community-based settings. This is not a remote position — you’ll deliver in-person care throughout your assigned area, Maricopa County, traveling to patient locations as needed.‎Â

Provide comprehensive primary and acute care to patients in Assisted Living, Memory Care, Hospice, and Group Home settings in Maricopa County.
Order, interpret, and evaluate diagnostic tests to assess and manage patients’ health conditions.
Deliver health education, counseling, and disease prevention guidance to promote wellness.
Collaborate with physicians, nurse practitioners, and other healthcare professionals as part of an interdisciplinary care team.
Develop, document, and update individualized care plans.
Prescribe medications and other appropriate treatments in accordance with licensure and scope of practice.
Requirements:
Requires an MS in Nursing
Minimum of 1 year of related experience with clinical emphasis for nurse practitioner; or any combination of education and experience, which would provide an equivalent background.
Current unrestricted RN license and NP license in applicable state required.
Satisfactory completion of a Tuberculosis test is a requirement for this position.
**This position is bonus eligible based on individual and company performance.**
