We’re hiring an Outpatient Clinicians to work with youth and adult clients in Walsenburg, CO. What You’d Be Doing Outpatient Clinicians at Health Solutions provide exceptional behavioral healthcare to include conducting assessments and screenings, generating and reviewing treatment plans, and providing individual, couples/family, and group therapies which support clients in moving toward improved health and well-being. Specifically, the Outpatient Clinician: Conducts screenings and assessments including those associated with substance abuse/mental health/co-occurring needs; Provides individual, couples/family, and/or group therapies; Provides case management services; Maintains accurate, complete, and up-to-date written and electronic records; Facilitates collaborative treatment planning with clients; Completes 90% of documentation within 24-hours of service delivery; Meets minimum monthly productivity standards; Maintains timely caseload management by progressing clients appropriately through treatment and discharging those who have completed treatment or are no longer engaged in a suitable manner; Participates in team meetings, case consultations, and other meetings as directed; Works collaboratively and represents Health Solutions with community agencies and partners; and Performs other job-related duties as requested and assigned based on program and client needs   Physical requirements include the following: May stay stationary for prolonged periods of time. Moves within the facility. Frequently exposed to stressful or emergent situations. Frequently operates a computer and other office equipment to record health data. Constantly communicates with clients and others. Occasionally moves equipment and/or materials up to 20 pounds.
