Role Overview: Nurse Practitioner - Part Time - Warrenton, VA

We are seeking a dedicated and compassionate Nurse Practitioner to join our Warrenton, VA team on a part‑time basis. This role provides an opportunity to make a meaningful impact on post‑acute patient care within skilled nursing and long‑term care facilities, working alongside facility staff to support resident‑centered outcomes.

Key Responsibilities

  • Provide high‑quality primary and acute medical care to residents in post‑acute and long‑term care settings

  • Perform timely on‑site assessments, diagnosis and management of acute clinical changes

  • Manage chronic conditions with a focus on preserving function, optimizing quality of life and reducing avoidable hospital admissions

  • Develop individualized care plans and set measurable goals in collaboration with residents, families and facility staff

  • Order and interpret diagnostic tests, initiate treatments and coordinate specialist referrals as required

  • Provide safe and effective medication management, including reconciliation and deprescribing when appropriate

  • Document patient encounters accurately and promptly in the Electronic Medical Record (EMR)

  • Collaborate with multidisciplinary teams through rounds, care‑planning meetings and family conferences to ensure coordinated, person‑centred care

  • Support facility quality and safety initiatives such as infection prevention, falls reduction and medication safety

Qualifications and Required Skills

  • Master’s degree or post‑graduate NP qualification from an accredited nursing programme

  • Active and unrestricted Nurse Practitioner license in Virginia and current DEA registration as required

  • Experience in post‑acute, long‑term care, gerontology or related settings preferred

  • Proficiency with Electronic Medical Record systems and accurate clinical documentation

  • Excellent clinical judgement, strong communication skills and the ability to work collaboratively within interdisciplinary teams

  • Current Basic Life Support (BLS) certification required; additional certifications (e.g. ACLS) desirable depending on facility needs

  • Compassionate, motivated and professional approach to resident‑centered care
