Part time life skills coach. Approx. 5 hours per week. Starting pay rate: $20.00 hour

OVERVIEW OF POSITION: Plans and conducts daily activities at theparticipant’s home to facilitate individual learning experiences which assistin the development of the participants’ living skills, to build independencyand integration with the community; consistent with the objectives identifiedin the Individual Service Plan (ISP)/Individual Program Plan (IPP), and withthe program design.

ESSENTIAL FUNCTION:

  • Plans and prepares a weekly schedule of activities that will provide learning of living skills and experiences according to the participant’s ISP and lPP goals and objectives.
  • Conducts daily activities, at the participant’s home, to assist in the development of living skills. Helps participants in building independence, to go out and integrate in the community, and carry out activities such as: do the chores of that day, prepare meals, do grocery shopping, utilize public transportation, attend doctor’s appointments, and learn financial management.
  • Provides 1:1 personal care for participants which may include: assistance with eating, personal hygiene, showering, restroom assistance, and preparing for bedtime.
  • Maintains all assigned documentation and data regarding participant including ISP's, quarterly and annual evaluations, and ID/case notes.
  • Assists in participant assessments, planning and evaluation processes. Attends ISP/IPP meetings. Assists participant in the development and implementation of his/her Service Plan.
  • Meets required training hour’s timely, utilizing internal and external training resources.
  • Performs other duties as assigned.

EDUCATION:

Typically requires High School Diploma, GED, or national equivalent.|Non-Violent Crisis Intervention (CPI), Professional Assault Response Training (PART) or Management of Assault Behavior (MAB) training. On-going certification required in CPI (note: required only of BMDT program).|Must possess and maintain valid CPR, CPI and First Aid Certifications.

EXPERIENCE:

  • 1 to 2 years’ experience, or equivalent combination of training, education and experience to perform the job successfully. Experience working with people in behavior management programs preferred.
  • Minimum age requirement is eighteen (18).

KNOWLEDGE, SKILLS, ABILITIES:

  • Ability to communicate well orally and in writing.
  • Ability to work independently, self-directed, when working at a participant’s home.
  • Ability to interpret and carry out written or oral instructions.
  • Ability to consistently demonstrate good judgment.
  • Working knowledge and understanding of Individual Program Plans and Individual Service Plans.
  • Ability to provide assistance with personal care to participants and the ability to transfer a program participant who has no weight bearing skills.
  • Ability to lift up to 50 pounds repetitively. Ability to walk, stand, stoop, and kneel for extended periods of time. Ability to push wheel chairs on a variety of terrain.
  • Ability to obtain and maintain a criminal record/fingerprint clearance from the Department of Justice and Federal Bureau of Investigation per Easter Seals Southern California and/or program requirements.
  • Ability to pass post-offer medical examination and test for tuberculosis.
  • Ability to travel locally. Appropriate vehicle for transportation of program participants, proof of valid California driver's license, clean driving record, proper auto insurance and vehicle registration, if program requires.
  • Ability to tolerate an outdoor work environment inclusive of a variety of weather conditions.
  • Ability to provide proof of required vaccinations or positive titer showing immunity. A signed declination may be acceptable for certain vaccinations.
  • Must pass all drug testing required by ESSC.

Carrying/Lifting: Occasional / 0-50 lbs.
Standing: Occasional / Up to 3 hours per day
Sitting: Constant / Up to 8 hours per day
Walking: Occasional / Up to 3 hours per day
Repetitive Motion/Activity: Keyboardactivity, telephone use, writing
Visual Acuity: Ability to view computer monitor and read newsprint
Travel: May require up to 20% local travel.
Environmental Exposure: Regular exposure to unpleasant or hazardous working Conditions (noise, heat, dust, bodilyfluids, etc.) more than 50% of work time.
