About the RoleÂ
The Recovery Coordinator – Adult provides outreach, information-referral, case management, and a full range of mental health and rehabilitation services for clients. This includes counseling, crisis intervention, skill training, and coordinating and monitoring assessment, treatment planning, and follow-up for each client. Level II Recovery Coordinators may act as lead staff, providing supervision and training to other recovery staff and volunteers. This role requires the use of a personal vehicle for client transport and active engagement with clients in home and community settings.Â
ResponsibilitiesÂ
· Conduct intake and comprehensive evaluation of each client’s life situation, needs, and challengesÂ
· Develop a detailed plan of care in collaboration with clientsÂ
· Assist clients in implementing their plan of care through community resource linkage and ongoing supportÂ
· Monitor the effectiveness of support services, including client and provider participationÂ
· Provide professional support through client visits, consultation, guidance, and temporary interventions as neededÂ
· Transport clients to accommodate community relationships (personal vehicle required)Â
· Maintain daily records, service progress notes, and billing forms per company and governmental standards (Short Doyle/Medi-Cal compliance)Â
· Conduct outreach and provide information/referral services to persons not requiring case managementÂ
· Maintain HIPAA compliance, ethical standards, and professional boundariesÂ
· Develop and maintain professional working relationships with agencies and service providersÂ
· Participate in management, staff, and client meetings, as well as training activitiesÂ
· Meet monthly productivity requirement of 100 hoursÂ
· Perform other duties as assigned to support the program and companyÂ
Required QualificationsÂ
· AA, AS, or BA degree in human services, psychology, or related field, plus one year of related experience OR two years’ equivalent experienceÂ
· Valid California driver’s license and auto insuranceÂ
· Current CPR certificationÂ
· Minimum one year prior case management or psychiatric treatment experienceÂ
· Excellent prioritization, organizational, interpersonal, and communication skillsÂ
· Intermediate PC skillsÂ
· Related experience (preferred): coaching, scouts, teacher’s aide, or CPS aideÂ
· Ability to complete six months of closely monitored trainingÂ
Preferred QualificationsÂ
· Knowledge of rehabilitation models and mental health case managementÂ
