MAJOR DUTIES:Ā 

The Case Manager Supervisor (CM Supervisor) supports the Program Director by assisting with designated administrative responsibilities and performing assigned case management duties. This role provides supervision, guidance, and professional development to the Permanent Supportive Housing (PSH) Case Managers, ensuring the delivery of high-quality, person-centered, and trauma-informed care to individuals experiencing chronic homelessness, often alongside co-occurring behavioral health and/or substance use disorders. The CM Supervisor ensures that all services provided are in full alignment with PSH program standards, contractual obligations, and evidence-based practices, fostering a culture of accountability, client empowerment, and service excellence. BRIEFĀ DESCRIPTION:Ā 

Supervision & Staff Support: Provide direct supervision to PSH Case Managers, including Monthly individual supervision, field observation, and group case conferencing/supervision weekly. Support staff in managing complex client cases, including those involving behavioral health crises, legal issues, or housing instability. Conduct performance evaluations and develop professional development plans. Approve and monitor caseload assignments in consultation with Program Director, home visit schedules, and documentation compliance.

Program Oversight & Compliance: Ensure compliance with programming policies, funding source regulations (e.g., HUD, Medicaid, DHS), and documentation standards.Ā Monitor Case Manager productivity and client engagement metrics address any gaps in performance. Conduct monthly chart and HTH audits to ensure the program meets DHS contractual requirements.Ā Lead case reviews and ensure Service Plans, Biopsychosocial Assessments, Intake documentation, LRSP applications, and Lease-ups are completed and updated in a timely manner. Serve along with Program Director as point of escalation for crisis intervention or high-needs cases.


Collaborate with external partners (e.g., landlords, behavioral health providers, medical facilities, and legal services) to support client outcomes. Oversee service delivery to ensure clients are progressing toward housing retention, income stabilization, and improved wellness.


Training & Quality Improvement: Provide or coordinate staff training on best practices, including harm reduction, motivational interviewing, Housing First, and trauma-informed care. Participate in agency leadership meetings, quality assurance initiatives, and case management planning. Identify areas for program improvement and implement strategies to enhance service quality and staff efficiency.

Administrative Responsibilities: Maintain accurate supervisory records and contribute to program reports and data tracking. Assist Program Director with hiring, onboarding, and training of new case management staff. Support grant and audit preparation as needed. Carry a small caseload max of 10. cases.

Team Collaboration: Collaborate with other members of the homeless services team, including outreach workers, housing specialists, employment counselors, and support staff, to ensure a coordinated and holistic approach to client support.

MINIMUM QUALIFICATIONS:

Master’s degree in social work, Human Services, or related field required; LGSW preferred. Minimum 5 years of case management experience in housing, mental health, or related field providing counseling and case management services to individuals/families experiencing homelessness or other related population. At least 2+ years of supervisory or team lead experience required. Previous experience in case management, preferably in homeless services or a related field. Knowledge of PSH models, Housing First principles, and supportive services delivery. Knowledge of community resources and social service agencies. Ability to work effectively with individuals from diverse backgrounds and cultures.Ā Familiarity with local homeless services systems, HMIS, and Medicaid billing Strong organizational, interpersonal, and crisis management skills. Proficiency with electronic documentation systems and Microsoft Office Suite.
