Concordia Hospice of Washington is a partnership between Concordia Community Support Services and Washington Health System that offers both quality home care and inpatient hospice end-of-life care. Concordia Hospice of Washington serves our patients in Washington and Greene counties with the focus to provide services that offer comfort, peace, and dignity for those with incurable illnesses and for their families. Hospice Medical Social Worker:

What you’ll do as a Social Worker:

• Provide emotional support, counseling, and education to patients and families
• Guide families through difficult decisions and connect them with community resources
• Advocate for patient needs as part of an interdisciplinary care team
• Support patients’ wishes, dignity, and spiritual well-being
• Help ensure that no patient or family ever feels alone in their hospice journey

Who you are as a Social Worker Social Worker:

• Master’s Degree in Social Work (MSW)
• A heart for service, empathy, and compassion
• Experience in hospice, healthcare, or counseling preferred
• Strong communication and listening skills
• A collaborative, team-focused spirit
