Patient Care Coordinator (Dental)

Position Summary

Unity Health Care is seeking a compassionate and organized Patient Care Coordinator (Dental) to support patients with complex health needs by coordinating dental services and assisting them in navigating the healthcare system.

The Patient Care Coordinator works closely with dental providers and interdisciplinary care teams to ensure patients receive timely, patient-centered care. This role conducts outreach, coordinates care plans, assists with scheduling and patient registration, and helps remove barriers that prevent patients from accessing dental services.

Key Responsibilities

  • Coordinate and manage dental care services for patients needing treatment

  • Conduct patient outreach and follow-up to support care coordination

  • Schedule and manage dental appointments while maintaining clinic workflow

  • Assist patients with registration, forms, and insurance verification

  • Verify insurance eligibility, benefits, referrals, and pre-authorizations

  • Maintain patient records and collect payments according to clinic policies

  • Support interdisciplinary care teams in managing patient care plans

  • Help identify barriers to care and connect patients with community resources

  • Serve as liaison between patients, providers, and support staff

  • Provide administrative and front desk support for dental clinics

Minimum Qualifications

  • High School Diploma or GED required

  • College coursework in business or health-related field preferred

  • Two (2) years of experience in care coordination or healthcare support

  • Experience working in a hospital, community health, or outpatient setting preferred

  • Experience working as part of an interdisciplinary healthcare team
