Senior LIFE is an innovative home and community based Medicare and Medicaid funded program which provides all-inclusive healthcare services and support to seniors living in the community. Senior LIFE makes it possible for seniors to remain at home, enjoying the comfort and reassurance of familiar surroundings while receiving the care they need.  Senior LIFE staff live the mission of the program.  Our team is committed to partnering with our seniors to remain at home through promoting open communication and shared decision making while providing excellent care and services.

Responsibilities include but not limited to: 

  • Identifying medical needs for individual members, and setting care priorities accordingly
  • Developing ongoing Plans of Care, in partnership with Senior LIFE interdisciplinary teams
  • Interpreting, recording and acting on diagnostic results, and performing advanced nursing techniques and critical care procedures
  • Identifying and assessing major medical problems, and communicating potential needs to a physician or specialist

Qualifications

Applicants should have at least one year working specifically with an elderly or aging population, hold a current RN license, and valid driver's license.
