Patient Care Coordinator – Internal & Family Medicine

Position Summary

Unity Health Care is seeking a Patient Care Coordinator – Internal & Family Medicine to support vulnerable and medically complex patients through care coordination, outreach, and patient navigation. This role collaborates with providers, nurses, and social service staff to ensure coordinated, patient-centered care across clinics, hospitals, and community settings.

Key Responsibilities

Coordinate care and provide patient navigation for high-risk patients

Support development and follow-up of individualized care plans

Coordinate services with hospitals, providers, specialists, and community organizations

Track patients following hospital discharge or emergency visits and ensure follow-up care

Assist patients with insurance, transportation, translation, and other barriers to care

Schedule appointments and coordinate specialty referrals

Conduct outreach in clinics, hospitals, homes, and community settings

Identify gaps in preventive and chronic care services

Participate in care team meetings and case conferences

Ensure compliance with HIPAA and Unity Health Care policies

Minimum Qualifications

High School Diploma or GED required

College coursework in healthcare, business, or related field preferred

2+ years of care coordination or patient navigation experience

Experience in hospital, outpatient, or community health settings preferred

Experience working on interdisciplinary healthcare teams

Required Skills

Knowledge of medical terminology and ICD-10

Familiarity with community health, discharge planning, and chronic disease management

Strong communication, organizational, and interpersonal skills

Proficiency with Microsoft Office and Electronic Medical Records (EMR)

Ability to work with diverse patient populations and travel between DC clinic locations
