Patient Care Coordinator – Internal & Family Medicine

Position Summary

Unity Health Care is seeking a Patient Care Coordinator – Internal & Family Medicine to support vulnerable and medically complex patients through care coordination, outreach, and patient navigation. This role works as part of an interdisciplinary care management team alongside providers, nurse care managers, and social service staff to ensure patients receive coordinated, patient-centered care.

The Patient Care Coordinator conducts outreach and care navigation across multiple settings throughout Washington, DC, including hospitals, primary care clinics, patient homes, homeless shelters, and community locations.

Key Responsibilities

Provide care coordination and patient navigation for vulnerable and high-risk patients

Develop, maintain, and support individualized patient-centered care plans

Use motivational interviewing techniques to build rapport and support patient health goals

Coordinate services between hospitals, primary care providers, specialists, and community organizations

Track patients following hospital discharge or emergency department visits and ensure follow-up care

Assist patients with insurance, transportation, translation services, and other barriers to care

Schedule appointments and coordinate specialty care referrals

Conduct outreach in clinics, hospitals, homes, and community settings

Identify and close gaps in preventive and chronic care services

Support panel management to reconnect patients lost to care

Participate in interdisciplinary case conferences and care team meetings

Provide culturally appropriate health education and support

Ensure compliance with HIPAA and Unity Health Care policies

Mentor site-based Care Coordinators and support care coordination processes across teams

Minimum Qualifications

High School Diploma or GED required

College coursework in business, healthcare, or related field preferred

2+ years of experience providing care coordination or patient navigation services

Experience in hospital, outpatient, or community health settings preferred

Experience working on interdisciplinary healthcare teams

Required Skills

Knowledge of medical terminology, ICD-10 codes, and healthcare procedures

Familiarity with community health, discharge planning, and chronic disease management

Strong interpersonal, organizational, and communication skills

Ability to work effectively with diverse patient populations

Proficiency with Microsoft Office and Electronic Medical Records (EMR) systems

Ability to analyze patient data and track care outcomes

Ability to travel between clinic locations and community sites in Washington, DC
