Are you an experienced Licensed Practical Nurse who wants to be a part of the care planning for your patient? As a Home Care nurse you will deliver one on one comprehensive care to a client requiring unique and complex clinical needs.

We currently have an opening for an Extended Care case, in a beautiful Waterbury home. This patient has been on services  with our agency for many years and will require you to have G-tube and Tracheotomy experience.

LPN must possess the following credentials in order to be considered for employment.

• Active CT Licensed Practical Nurse in good standing.
• At least one year of nursing experience
• Valid CT drivers license and reliable transportation.
• CPR certification
• G-tube and tracheotomy experience

Responsibilities:

1. Understands and adheres to established policies and procedures.

2. Provides nursing services, treatments and diagnostic and preventive procedures as assigned.

3. Observes signs and symptoms and reports to the physician and RN reactions to treatments, including drugs and changes in the patient's physical or emotional condition.

4. Teaches and counsels the patient and family/significant others regarding the nursing care needs and other related problems of the patient at home.

5. Evaluates with registered nurse the effectiveness of the LPN's/LVN's nursing service to the patient and family under the guidance of the registered nurse.

6. Maintains accurate and complete records of observations, treatments and care of patient.

7. Prepares clinical and progress notes.

8. Assists the physician and RN in performing specialized procedures.

9. Prepares equipment and materials for treatments.

Requirements

• Graduate of a state approved school of practical (vocational) nursing and currently licensed in the state(s) in which practicing
• Minimum of one (1) year experience in nursing preferred.
• Acceptance of philosophy and goals of this Agency.
• Ability to exercise initiative and independent judgment.
