Position Type/Expected Hours of Work – 40 hours per week; based in a transitional residential setting with regular interaction with clients, staff, and community partners. The role may require some evening or weekend work to meet program needs. 

 

Qualifications –Must be licensed and currently registered with the New York State Department of Education as a Registered Nurse. Experience working with individuals with dual diagnosis of mental health and developmentally disabled preferred. Valid New York State Driver’s License.   

 

Work Authorization/Security Clearance – Authorized to work in the United States.

 

Summary/Objective – This position focuses on integrating health services and community resources to improve health outcomes and enhance the quality of life for individuals with complex mental health and intellectual/developmental disabilities. The RN is responsible for assessing basic health and medical needs within their scope of practice, coordinating health care and acting as a liaison with community medical providers, rehabilitation, education, and support services for program participants. The RN will work directly with clients providing wrap around services for a small case load and work in conjunction with the team to address client medical needs of all program participants, so participants can acquire the skills needed to live independently in the community and effectively utilize community supports.

 

Essential Functions – Reasonable accommodations may be made to enable individuals with disabilities to perform the essential functions.

  1. Monitor the health and well-being of up to 30 participants in the ESD Program.
  2. Attend pre-admission, intake, discharge, and weekly meetings; review medical records, including current medications and treatments
  3. Obtain and coordinate medical services for participants with other agencies and review all physician/medical consult forms after medical visits, assure follow up regarding provider recommendations/prescriptions
  4. Provide oversight of psychiatric medications, providing written reports to the treating psychiatrist.
  5. Support client to access and effectively utilize community resources, including facilitating and attending meetings. 
  6. Advocate for clients, working to reduce barriers to services and increase access to resources.
  7. Monitor client progress and outcomes and adjust plans as needed to support successful transitions to permanent housing and stabilization of mental health.
  8. Deliver personalized, culturally competent, and trauma-informed care to all program recipients.
  9. Assist in training Direct Support Professionals on topics related to mental health medication and health.
  10. Any other duties as assigned by the CTI Team lead and/or Director of OMH services.

Travel – This job may require travel within the community to complete assessments, attend meetings or conferences. 

 

Work Environment – This job operates in a specialized residential setting with designated office space for management and clinical staff. This role routinely uses standard office equipment such as computers, phones, photocopiers, filing cabinets and fax machines
