Job DetailsJob Location: Watsonville, CA 95076Salary Range: $30.00 - $40.00 HourlyAbout Si Se Puede: Located in the serene foothills of Watsonville, CA, Si Se Puede is one of the only bilingual and monolingual treatment environments in the nation.  This 30-bed residential program serves Spanish speaking, male clientele for a one to six-month period, based on individual client needs. Staffed by a fully bilingual team, highly skilled, trained and certified clinicians help clients focusing on issues surrounding recovery from substance use and co-occurring disorders. Program activities are evidence-based and trauma informed, and include withdrawal management, cognitive behavioral therapies (CBT), focused individual and group counseling, pro-social life skill building, mindfulness, and daily house meetings. Job Summary: We currently have two full-time (one 40 hours/week and one 30 hours/week) openings for nurses at our substance use disorder residential treatment program, Si Se Puede. Hours are 10pm-8am and roles are not required to be on-call. The 40 hour/week LVN will cover Sunday- Wednesday and the 30 hour/week will cover Thursday-Saturday. Pay is dependent on experience. Under supervision of the Director of Nursing and onsite Family Nurse Practitioner, the LVN provides skilled nursing services within scope of practice to residents in treatment for substance use disorders. The nurse in this position performs ongoing assessments, supports with intakes and discharges, and provides education to residents and staff.  The nurse works closely with the MAT team to support residents with Medication Assisted Treatment, stabilization and monitoring. The nurse collaborates with multi-disciplinary team members within the organization and multiple healthcare settings within the community. We are looking for a nurse who is passionate about making a difference in our residents’ lives and in our community. Primary Responsibilities: In collaboration with a team delivering integrated behavioral health treatment services, supports onsite FNP in obtaining initial history and physical examination. Performs basic nursing services within scope of practice, such as: assessments and support in primary care physician (PCP) and dental care establishment and follow-up. Supports with referral coordination. Assists clients in improving and maintaining stability by providing medication support, education and monitoring. Ensures accurate implementation of prescribing provider orders, as well as safe adherence to medication protocols. Works closely with Medication Counselor and other identified trainers in supporting staff with medication curriculum. Participates in monthly Medication CQIW and Medication Training Workgroups. Participates in monthly medical team meetings and supervision. Follows prescribing provider orders, including placing and reading PPDs, monitoring and recording vital signs. Adheres to required infection control and aseptic techniques in the delivery of resident care. Communicates closely with onsite FNP and community healthcare providers to deliver integrated care and improve continuity. Observes, records and reports signs, symptoms, and behaviors including the physiological and psychological status of residents. Notifies appropriate supervisory staff of changes in residents’ status; recognizes care priorities and potential crises. Facilitates weekly health groups. Provides assessment, education and linkage to residents looking to access MAT treatment programs. Exhibits non-judgmental, empathetic and supportive approach when communicating. Meets MediCal billing requirements through appropriate documentation on EHR. Consistently uses clear, concise, and effective written and oral communication; complies with departmental policies when sharing/documenting relevant resident care data; promotes strict resident confidentiality and right to privacy. Works with the pharmacies, the onsite FNP and the MAT team in obtaining medication orders, prescriptions, refills, and prior authorizations. Consults with provider and/or RN whenever necessary in order to advise resident of most appropriate action; reinforces provider’s plan of care. Determines if resident needs to be seen by a provider and how quickly; schedules urgent appointments appropriately. In case of referral to the Emergency Department or another facility, coordinates care with appropriate staff at the other facility, including nurse-to-nurse reports. Actively participates in protocol development for the program with supervisor. Provides COVID-19 testing as needed to residents and staff. Other responsibilities as assigned by supervisor. Required Qualifications:  Successful completion of a nursing program. Unencumbered and unrestricted license to practice as a Licensed Vocational Nurse (LVN) or Registered Nurse (RN) within the state of California. Current BLS for Healthcare Providers certification. Must have a health examination including a screening for tuberculosis within 90 days prior to employment or within 7 days after employment and at least annually thereafter. Skilled in basic computer skills. Demonstrate skill in effective written and verbal communication. Ability to recognize the needs and concerns of diverse groups of people. Must be able to pass a criminal record clearance. Preferred but not required qualifications: Bilingual in Spanish candidates are encouraged to apply. 1 year or more experience working as a nurse.
