Care Coordinator (Scheduler & Charge CNA)

Harwood Place is seeking an experienced CNA to join our team as our Clinical Services Care Coordinator! The Care Coordinator will function in a dual role as both a Charge Resident Assistant and the Nursing Scheduler. Interested candidates should be organized, outgoing and dedicated to our mission and “4 C’s.”

Hours for the position are Monday - Friday 10:00 AM – 6:30 PM.

Requirements for the position:

• Certified Nursing Assistant (CNA) in the state of Wisconsin
• Current or willingness to train for CBRF certifications in the state of Wisconsin
• Ability to develop and maintain professional working relationships with peers
• Proficiency with Microsoft Office and electronic health records
