This position offers the opportunity to provide meaningful, patient-centered care across home health and outpatient therapy in the home, working with older adults in a variety of settings. Our SLPs play an important role in helping patients maintain independence, improve quality of life, and achieve their functional goals. Looking for a clinician who is confident in navigating complex patient presentations, fast-paced transitions of care, and interdisciplinary collaboration. You will have the opportunity to practice at the top of your license while delivering high-quality, patient-centered care across multiple levels of care. Qualifications Graduate from an accredited school with a master’s degree in Speech-Language Pathology. Licensed by the State of Wisconsin to practice as a Speech-Language Pathologist.
