General Description: Accomplishes client care by assessing treatment needs; developing, monitoring and evaluating treatment plans and progress; facilitating interdisciplinary approaches, monitoring client performance.
Position Qualifications:
- High School Diploma or GED
- 3+ years customer service experience, preferably in human services, possess demonstrated commitment to customer service
- Professional or personal experience supporting individuals with disabilities, mental illnesses, or challenging behaviors highly preferred
- Must have a valid in-state driver’s license, provide current registration and insurance, and have a driving record with no more than 6 points

Working Conditions:
General office environment. Employee is frequently required to walk, sit, stand or kneel and occasionally required to climb or balance and stoop. Employee must occasionally lift and/or move up to 25 pounds. Must have ability to sit for long periods of time at a computer. Lighting and temperature are adequate, and there are no hazardous or unpleasant conditions caused by noise, dust, etc. Employee must be able to travel between Unison sites. Performance OBJECTIVES & GOALS:
Assessment & Case management
Determines client's needs by completing intake interviews, determining need for therapeutic, medical, psycho-social, and psychiatric evaluations; reviewing therapist evaluations, treatment objectives, and plans. Provide case management services for set number of individuals to assist with caseload.
- Establishes treatment programs by setting schedules and routines; coordinating services being provided; arranging resources, including transportation and escort.
- Monitors cases by verifying clients’ attendance; observing and evaluating treatments and responses; advocating for needed services and entitlements; obtaining additional resources; intervening in crises; providing personal support.
- Maintains clients’ records by reviewing case notes; logging events and progress.
- Assists with the development of an individual service/treatment plan for each assigned client.
- Maintain strict confidentiality of information at all times according to established departmental policies & procedures.
- Adjust techniques & interventions as needed to ensure barriers to progress are addressed and movement toward recovery/independence is facilitated.
- Ensure that all treatment plans are current and services are authorized before being provided.
- Deliver services that are clinically appropriate to client's needs as specified in the treatment plan and meet responsibilities to client and third party payor by providing only services that meet best practice guidelines.
- Oversee the discharge planning process.
- Ensure assigned reports are submitted monthly.
- Ensure billing for provided services are loaded, accurate and current.

This will be done by ensuring documentation is completed concurrently and utilizing Eleos Behavioral Health software as applicable.
Documentation and Compliance:
- Ensure all documents are received/loaded into system/contain correct information for all services provided.
- Records services accurately that relate directly to the treatment outcomes, within approved timeframes. Completes required clinical documentation according to agency standards.
- Maintain all documentation in accordance with applicable policies, laws and instructions.
- Ensure that all services provided are within the guidelines and document care in compliance with agency requirements and standards.
- Ensure that all notes will be in CareLogic and signed no later than 10:00 A.M. the day after the service was provided.
- Maintain a minimum chart audit score of 93% or better for all consumers on case-load.
- Ensure all weekly reports have been addressed and cleaned up (if necessary) within 48 hours of receiving reports.

Community Outreach
Facilitates relationships between Unison, service user families/legal guardians and various social service community resources, such as housing assistance, healthcare, job training and placement and substance abuse support groups.

Treatment Team
- Communicates clients’ progress by conducting weekly interdisciplinary meetings and evaluations; disseminating results and obstacles to therapeutic team and family; identifying treatment influences. Participates in case reviews, consulting with the team to evaluate the client's needs and plan for continuing services.
- Attends and participates in the interdisciplinary treatment planning.
- Attend unit treatment team meetings, unit meetings and supervisions.
- Coordinates community resource availability based on client needs (day program, guardians, etc.) to assure maximum social integration.
- Read and review treatment team notes weekly.

Works when Scheduled
Submit all non-medically related PTO requests in to supervisor a minimum of 30 days in advance. Submit all medically related scheduled PTO requests in to supervisor a minimum of 7 days in advance. Submit all emergency PTO leave request (i.e., call in because of the flu, stomach virus, etc.) in to supervisor the day of return to work.
