Job DetailsJob Location: Wayne County, MI - Wayne, MI 48184Registered Nurse Case Manager (RNCM) Manageable caseloads, flexibility, and meaningful hospice work that allows you to focus on quality patient care.   The Registered Nurse Case Manager supports hospice patients receiving care in their homes or facility settings by providing skilled nursing care and coordinated, compassionate end-of-life support in partnership with the interdisciplinary team. Schedule, Hours & Territory  • Standard operating hours are Monday through Friday, 8:00 AM to 4:30 PM  • Wayne County, MI  • RN Case Managers may participate in an on-call rotation with additional pay  • Some branches offer dedicated on-call staff, resulting in backup on-call coverage only What We Offer  • Strong interdisciplinary team collaboration  • Supportive clinical leadership and resources  • Clear expectations and consistent workflows  • Environment focused on quality, compliance, and patient-centered care Benefits  • Medical, dental, and vision coverage  • 401(k) with employer match at 100 percent on the first 3% and 50 percent on the next 2%  • Health Savings Account eligibility with employer contribution up to $500 for individual coverage and $1,000 for two-person or family coverage  • Company-paid Life and AD&D  • Voluntary disability, accident, critical illness, and hospital indemnity options  • Employee Assistance Program, WillPrep, and travel assistance resources  • Tuition reimbursement  • Paid time off and paid holidays  • Mileage reimbursement per company policy Key Responsibilities  • Conduct comprehensive patient assessments and develop individualized plans of care  • Provide skilled nursing care in accordance with hospice standards and physician orders  • Coordinate care with physicians, caregivers, and interdisciplinary team members  • Educate patients and families on symptom management, disease progression, and end-of-life care  • Document assessments, interventions, and care plans accurately and timely in the EMR HCHB (Homecare Homebase)  • Participate in interdisciplinary team meetings and care planning  • Support after-hours patient needs as part of an on-call rotation when applicable Qualifications   • Active Registered Nurse (RN) license  • Hospice or home health experience preferred  • Strong clinical assessment and care coordination skills  • Ability to work independently in the field  • Strong communication and documentation skills  • Experience using HCHB (Homecare Homebase) preferred Field Role Requirements  • Active driver’s license  • Ability to pass a criminal background check  • Ability to pass a Motor Vehicle Record (MVR) check  • Current TB testing in accordance with agency policy  • Current CPR certification as required for the role
