Registered Nurse (RN) – Home Health

Pay Rate: $55–$59/hr
Mileage Reimbursement: $0.45 per mile
Schedule: Monday–Friday (Flexible)
On-Call: 1 week per month (easy rotation)
Coverage Area: Charlottesville to Staunton, VA


Position Overview

We are seeking an experienced Registered Nurse (RN) to provide high-quality, patient-centered home health services. The ideal candidate will be comfortable managing a flexible schedule, completing required daily productivity points, and collaborating with an interdisciplinary team to ensure exceptional outcomes and continuity of care.


Primary Responsibilities

  • Conduct comprehensive assessments for high-risk members and patients in their home setting.

  • Collaborate with primary care providers and other healthcare professionals to develop and implement individualized, goal-oriented care plans.

  • Identify and coordinate cost-effective, appropriate levels of care across the continuum.

  • Facilitate non-medical supports (housing, transportation, etc.) to promote adherence to care plans.

  • Manage and coordinate care transitions to ensure seamless continuity between providers, facilities, and home settings.

  • Partner with physicians, social workers, discharge planners, and claims professionals to support optimal patient outcomes.

  • Maintain detailed documentation of clinical, functional, and financial outcomes.

  • Identify opportunities for health promotion, illness prevention, and patient education.

  • Act as a patient advocate while maintaining strict confidentiality and HIPAA compliance.

  • Support the prevention and early intervention of adverse patient outcomes.


Qualifications

  • Education: Associate’s Degree in Nursing (BSN preferred).

  • Licensure: Current, unrestricted multi-state RN license (Compact).

  • Experience: Minimum of 1–2 years of clinical experience; home health experience preferred.

  • Certifications:

    • Current CPR/BLS certification

    • TB test clearance

    • Wound care/competency test completion

    • Case Management Certification preferred

  • Technical Skills: Proficiency with OASIS documentation and Homecare Homebase (HCHB) required.

  • Strong analytical and decision-making skills in assessing complex medical situations.

  • Excellent communication, time management, and patient engagement abilities.
