Job PurposeÂ
Collaborating with providers and multidisciplinary team members to deliver appropriate client care. Assist with the coordination of the psychiatrist’s/nurse practitioner’s daily activities. Â
QualificationsÂ
- Must be licensed LPNÂ
- Psychiatric/Behavioral Health and Healthcare technology experience preferredÂ
- Ability to perform basic nursing assessments        Â
Core Job DutiesÂ
- Perform vitals. Â
- Perform screenings per Prescriber’s request. Â
- Weekly updates on sample medication inventory and storage as well as quarterly inventory report with Registered Nurse (RN) assistance. Â
- Actively working refill request from pharmacy(s). Â
- Perform blood/urine collection(s) as requested by Prescriber(s). Â
- Request lab results and follow up with reported results. Â
- Call in prescriptions and refill medications as needed and directed by the Prescriber. Â
- Manage written prescription(s) documentation. Â
- Manage medication injection(s) and medication injection(s) documentation. Â Â
- Manage Patient Assistance for all medications for the Prescribers. Â
- Complete prior authorizations for Medicare/Medicaid/insurances for the Prescribers. Â
- Answer phone calls/voice mail(s) regarding medications and notify Prescriber of adverse reactions. Â
- Actively responds to emails regarding medications for person(s) served. Â
- Handle request for records/discharge summaries as requested by Prescriber. Â
- Perform metabolic screenings for person(s) served. Â Â
- Administer Tuberculosis (TB) testing for staff. Â
- Orally communicate information effectively and accurately. Â
- Assure program operates within the constraints of the agency’s certification (CARF, DMH) standards and strategic plan related to clinical care.  Â
- Abide by program and agency policies and procedures.Â
- Other job duties and special projects as assigned.Â
