Schedule: Monday-Friday 8am-5pm
Pay Range: $90,000-95,000 per year, based on experience
Location: This position will be based out of our Partners in Health office in Westminster and will travel between all 4 of our clinics. Partners in Health, Westminster, Arvada, & Lakewood

Essential Duties:
- Provide exceptional behavioral health interventions, within an integrated care setting, utilizing therapeutic intervention models (brief solution focused, CBT, MI, ACT etc.)
- Experience and willingness to work with a diverse population with a wide range of mental health challenges, including substance abuse.
- Work effectively and collaboratively with the primary care medical providers to deliver whole person care
- Coordinate care with other community providers and provide linkage and referrals to other services as needed.
- Set and monitor achievable clinical goals and outcomes
- Participation in provider team meetings.
- Ability to provide both in person and telehealth treatment.
- Willingness to explore other clinical interventions that may benefit the patients of the practice (i.e., clinical groups or other modes of providing care as deemed appropriate and beneficial)
- Timely documentation
- Other duties as assigned
- Master's Degree (from accredited university/college) in Social Work
- License required: LCSW
- Self-motivated and the ability to work independently as well as within a team
- Experience coordinating care within the community to wrap around services that may not be available within the primary care setting.
- Ability to assess and appropriately manage crisis situations that require risk evaluation and management.
