Job Summary:

School Based Community Health Workers (SBCHWs), work alongside school personnel and school-based Nurse Practitioners to help students and families access medical and behavioral health services and community-based resources. CHWs provide strong care coordination and help students access resources to support health, personal and academic achievement. Community Health Workers offer a warm and supportive approach to providing care coordination, family communication, insurance enrollment support, and work with the student and families to access resources in the best interest of the student and to strengthen family support. Maintains enrollment and visit records for the School Based Health Center.

Responsibilities

Functional Demands

Exerts up to 20 pounds of force occasionally, and/or up to 10 pounds of force frequently, and/or a negligible amount of force constantly to move objects.  Frequently reaches (extending hands and arms in any direction), and handles (seizing, holding, grasping, turning, or working with hands).

Essential Functions

  • Administer SBHC enrollment forms and actively manage annual enrollment.
  • Supports the NP in the coordination of care, obtaining insurance information and PCP referrals.
  • Assist students and families to access on site or tele medical, behavioral, and community based resources (as needed).
  • Provides administrative support to the NP including: coordinating meetings, prepares materials, attends meetings and records minutes, assists with data collection and reporting and assists with special projects, promotions and events.

  • Monitors student waiting to ensure safe environment and identifies and removes hazards.
  • Maintain data on each student /family referred to the program and monitors tracker to ensure each referred student receives appropriate services.
  • Provide home, school and community-based care coordination to youth and their families.
  • Help students remain in the school setting and academically focused.
  • Become a member of the Student Assistance Team in each school to discuss opportunities to improve student access to services and to address prevalent needs.
  • Manage and schedule medical appointments with the SBHC NP and behavioral health appointments with the SB Mental Health Clinician.
  • Work on special projects to include opportunities for program growth to enhance program success.
  • Participate in weekly supervisions and bi-monthly School Based Health Center meetings.
  • Provide regular and ongoing communication and engage with community partners
  • Become a subject matter expert, (SME) in the knowledge of local community resources.
  • Attend relevant training, conferences, community events.
  • Maintain confidential information.
  • Follow Heywood’s policies, and procedures.
  • Participate in creating and maintaining a workplace that values diversity.
  • Manage time and resources in a skilled manner
  • Demonstrate appropriate boundaries in all interactions.

Qualifications

Job Requirements 

 

Minimum Education

  • Associate’s Degree, Community Health Worker Certification, or 5 years related experience required.

Required Skills

  • Medical office experience, solid computer skills, attention to detail, and ability to multi-task required.

  • Excellent communications and interpersonal skills required to foster relationship building with internal and external stakeholders (including the school community, students and families).
