JOB SUMMARY: The Development & Grants Coordinator plays a key role in advancing the mission and long-term sustainability of HOPE Family Health through strategic grant development, fundraising support, donor engagement, community partnerships, and organizational storytelling. Working closely with executive leadership, this position helps strengthen organizational visibility, diversify funding opportunities, and cultivate relationships that expand HOPE’s impact across the communities it serves.

This position is responsible for coordinating and supporting all phases of the grant development process, including researching, writing, editing, submitting, tracking, and reporting on Federal, State, local, and private foundation grant opportunities. The Development & Grants Coordinator must possess strong writing, organizational, and project management skills with the ability to manage multiple grant timelines, collaborate across departments, and communicate complex organizational and programmatic information clearly and effectively.

In addition to grant coordination, this position supports fundraising and donor stewardship activities, community engagement initiatives, sponsorship development, and strategic communications efforts that strengthen philanthropic support and community partnerships for HOPE Family Health. The Development & Grants Coordinator serves as a relationship-builder and storyteller who helps connect HOPE’s mission, patient impact, and strategic priorities to funders, donors, community organizations, and the public.


PRIMARY DUTIES & RESPONSIBILITIES:

  1. Grant Development & Management
    1. Research, identify, and coordinate Federal, State, local, and private foundation grant opportunities aligned with HOPE Family Health’s mission, strategic priorities, and organizational needs.
    2. Coordinate all phases of the grant development process, including proposal writing, editing, budgeting collaboration, submission, tracking, and required reporting.
    3. Collaborate with executive leadership, finance, clinical leadership, operations, and program staff to gather data, narratives, outcomes, and supporting documentation for grant applications and reports.
    4. Maintain and manage a grants calendar to ensure timely submission of proposals, reports, renewals, and compliance requirements.
    5. Assist in tracking grant outcomes, deliverables, reporting deadlines, and funding utilization to support organizational accountability and sustainability.
    6. Maintain organizational grant files, templates, funder records, and supporting documentation.
    7. Develop, maintain, and manage an organizational grant pipeline that tracks Federal, State, local, and foundation funding opportunities from identification through submission, award management, and reporting.
  2. Fundraising & Donor Engagement
    1. Support fundraising initiatives that strengthen philanthropic giving and diversify organizational revenue streams.
    2. Assist with donor stewardship activities including donor communication, acknowledgments, sponsorship coordination, and relationship cultivation.
    3. Support annual giving campaigns, special fundraising initiatives, and community sponsorship opportunities.
    4. Assist in developing materials and storytelling content that communicate HOPE’s mission, impact, and community needs to donors and supporters.
    5. Help strengthen and promote the HOPE Assistance Fund and other charitable giving initiatives.
    6. Support development efforts related to future organizational growth, capital projects, strategic expansion initiatives, and community investment opportunities.
  3. Community Relations & Partnerships
    1. Build and maintain positive relationships with community partners, foundations, civic organizations, businesses, and other stakeholders throughout HOPE’s service region.
    2. Coordinate and participate in outreach events, speaking engagements, community meetings, and partnership opportunities that strengthen HOPE’s visibility and mission awareness.
    3. Represent HOPE professionally at community events and engagement opportunities as assigned.
    4. Assist in cultivating strategic partnerships that support organizational growth, patient access, and community impact.
  4. Communications & Storytelling
    1. Assist with development of annual report, newsletters, donor communications, grant narratives, social media content, presentations, marketing materials, and organizational storytelling initiatives.
    2. Help maintain consistent messaging, branding, and mission-focused communication across development and outreach efforts.
    3. Collect and organize patient stories, program outcomes, organizational impact data, and community success stories that support grant applications, fundraising efforts, and public communications.
    4. Assist in maintaining development-related content on organizational websites and communication platforms.
  5. Organizational Support & Strategic Collaboration
    1. Work collaboratively with leadership and departments across the organization to identify emerging funding opportunities, community needs, and strategic development priorities.
    2. Participate in organizational planning, development discussions, and strategic initiatives related to sustainability, outreach, fundraising, and growth.
    3. Maintain confidentiality regarding organizational, donor, patient, and employee information.
    4. Perform other related duties as assigned by executive leadership.


INTERMITTENT DUTIES:

  1. Assists with organizational fundraising events, donor engagement activities, community outreach initiatives, and strategic development projects as needed. 
  2. Participates in organizational planning, community engagement, and cross-department collaboration efforts related to development, communications, outreach, and sustainability initiatives. 
  3. May assist leadership with special projects, presentations, reports, campaigns, partnership initiatives, and organizational storytelling efforts. 
  4. Performs other related duties as assigned by executive leadership.


OFF-SITE WORK:

Off-site work is required for this position as part of community engagement, outreach, fundraising, grant-related activities, and organizational representation responsibilities.

  1. Required Off-Site Work
    1. The Development & Grants Coordinator is frequently required to attend community events, meetings, conferences, trainings, outreach activities, fundraising events, partnership meetings, and other organizational functions that occur off-site. Travel within HOPE’s service region and occasional travel outside the region may be required. This position may occasionally require evening or weekend participation for community engagement activities, fundraising events, grant deadlines, or organizational initiatives.
  2. Work Location Expectations
    1. This position is primarily an on-site position based within HOPE Family Health facilities. Due to the collaborative, relationship-centered, and community-facing nature of the role, routine remote work is not an expected component of this position. Exceptions for temporary off-site administrative work may be considered on a limited basis at the discretion of executive leadership and organizational needs.


SKILLS/QUALIFICATIONS:

  1. Education
    1. Bachelor’s degree in communications, public health, nonprofit management, business, marketing, journalism, healthcare administration, English, social services, or a related field preferred.
    2. Equivalent combinations of education, relevant experience, and demonstrated success in grant writing, fundraising, communications, or nonprofit development may be considered.
  2. Experience
    1. Minimum of three (3) years of experience in grant writing, nonprofit development, fundraising, communications, community relations, healthcare administration, or a related field preferred.
    2. Demonstrated experience researching, writing, coordinating, and managing grant applications and reports strongly preferred.
    3. Experience working with Federal, State, local, and/or private foundation grants strongly preferred.
    4. Experience working in healthcare, nonprofit, rural health, public health, behavioral health, or community-based organizations preferred.
    5. Experience supporting fundraising initiatives, donor engagement, sponsorships, or community partnerships preferred.
  3. Knowledge, Skills, & Abilities
    1. Excellent written, verbal, editing, and proofreading skills with strong attention to detail.
    2. Ability to write and coordinate multiple types of grant proposals including Federal, State, local, and foundation applications.
    3. Strong organizational and project management skills with the ability to manage multiple deadlines and competing priorities.
    4. Ability to gather, organize, and communicate complex organizational, programmatic, financial, and outcome-related information clearly and effectively.
    5. Strong interpersonal and relationship-building skills with the ability to professionally engage donors, funders, community partners, leadership, and the public.
    6. Ability to work collaboratively across departments and maintain positive working relationships.
    7. Ability to maintain confidentiality regarding organizational, donor, employee, and patient information.
    8. Ability to work independently while managing multiple ongoing projects and responsibilities.
    9. Demonstrated commitment to HOPE Family Health’s mission and service to underserved populations.

    2. Ability to learn grant management, donor management, and communication platforms as needed.
    3. Strong professional judgment, problem-solving, and organizational skills.
    4. Valid driver’s license and ability to travel within HOPE’s service region as needed.
  6. Physical Requirements
    1. Requires the ability to communicate effectively verbally and in writing.
    2. Requires sufficient visual acuity to read written materials, review documents, operate a computer, and prepare reports and grant applications.
    3. Requires prolonged periods of sitting, computer work, writing, and data entry.
    4. Requires occasional standing, walking, bending, reaching, and lifting of materials up to 25 pounds.
    5. Requires the ability to travel to meetings, community events, conferences, and outreach activities as needed.
