This position plays a critical role in supporting client access to behavioral health, medical, and community services by ensuring timely, courteous, and reliable transportation while maintaining confidentiality and professionalism. The ideal applicant for this position is one who has excellent organizations skills and time management. This includes the ability to work with others as a team they will be coordinating with other staff to arrange transportation for clients. This role ensures clients receive safe, timely, and appropriate transportation that supports their access to care and overall well-being. This position will be conducted remotely. ESSENTIAL DUTIES: Safely transport clients to and from their appointments at Valley Healthcare System in accordance with transportation requests submitted on the client’s behalf. Ensure client safety during transport, including proper use of seat belts and adherence to all traffic laws and agency policies. Maintain confidentiality and comply with HIPAA and Valley Healthcare System standards and procedures. Assist clients with entering and exiting the vehicle as needed, while maintaining professional boundaries. Conduct pre- and post-trip vehicle inspections and report maintenance or safety concerns promptly. Communicate effectively with program staff regarding scheduling changes, delays, or client concerns. Follow agency policies related to safety, infection control, and emergency procedures. Confirm transportation arrangements directly with clients, families, or caregivers, and provide instructions and updates as needed. Work with case managers, customer support, clinical staff, etc. to coordinate and facilitate transportation for clients. Maintain accurate transportation logs, mileage records, and client pickup/drop-off documentation. This includes completion and submission of Modivcare transportation logs in a timely manner. Use clear communication skills, including “people first language” with consumers and other professionals. Complete and submit program specific documentation in a timely and accurate manner. Monitor, maintain, and use appropriately, all equipment and supplies. Completes and submits Electronic Medical Record documentation as required by Valley, OHFLAC, BMS, CMS, and other licensing agencies completed accurately and timely and will accurately and appropriately reflect services the consumer(s) has received. Attends in-service trainings, mandated meetings and completes Essential Learning trainings as assigned. Adheres to Valley’s standards of conduct and compliance and follows ethical guidelines of their profession. Participates in group or individual supervision with the supervisor. Promotes appropriate community interactions by handling public interactions in a professional and competent manner. Returns telephone calls and requests for information in a timely manner, promotes a positive image of the agency, and maintains a good rapport with agency contacts. Other duties as assigned. QualificationsMINIMUM QUALIFICATIONS: Ability to perform Essential Duties as outlined below. High School diploma or equivalent. Ability to communicate effectively both verbally and in writing with clients and individuals at all levels within the organization. Ability to work with limited direct supervision. Strong organizational skills. Ability to comply with all of Valley’s policies and procedures. Ability to comply with Client’s Rights. Ability to comply with Valley’s and division/departmental safety procedures. Ability to read, write, understand, and speak the English language. Working knowledge of computer applications including Microsoft Outlook, Teams, and Word.
