Job Description
The Care Manager (Caregiver) assists residents in all aspects of their daily life as indicated in the resident’s care plan, including personal care, food service, housekeeping, laundry, behavior management, socialization, and activities. This position is a non-exempt, hourly position reporting to the Health & Wellness Director.
Salary Range: $21.00 - $23.00 Houly
Responsibilities and Duties:Â
- Provide resident care to residents that include bathing, dressing, toileting, and feeding, as outlined in their care planÂ
- Respects the needs of team members and residents by adhering to schedule and documenting attendance properly.Â
- Maintain the safety, comfort, dignity, and confidentiality of all residentsÂ
- Encourages and escorts (when needed) residents to meals and activitiesÂ
- Ensures all assigned residents are clean, neatly dressed, groomed, safe, and fed during assigned shiftsÂ
- Document all services provided in ALIS EHR for your assigned residents during your shiftÂ
- Monitors and reports any change of condition in a resident immediately to your supervisor Â
- Appropriately utilizes and keeps electronic documentation “devices” safe and secure within the community; efficiently documents care/services provided during the shift.Â
- Participates in and attends all in-service trainings as scheduledÂ
- Ensures compliance with all Federal, State, and local regulations, as well as community policies and proceduresÂ
- Provides basic housekeeping and/or laundry services when neededÂ
- May be asked to obtain weights and vitals of residents and document same per RN requestÂ
- Other duties as assigned by supervisor, RN, and/or EDÂ
Qualifications
- High School Diploma or GED. At least 1 year resident care experience required.Â
- Certified Nursing Assistant or Certified Home Health AidÂ
- Knowledge and experience in working with the elderly population, preferred dementia experienceÂ
Physical Abilities:Â
- While performing the duties of this job, the associate is often required to stand, walk, sit, use fine and gross motor skills, reach with hands and arms, balance, stoop, kneel, crouch, talk, hear, and smell. Â
- An individual in this position will be required to lift or carry weight in up to 50 lbs. Intermittent physical activity includes lifting and supporting residents. The associate must use proper body mechanics.Â
