Position Title: Care Coordination Specialist
Department: Clinical Operations 
Status: FTE 1.0 
Schedule: Full-time, Monday–Friday, 8:00 am – 4:30 pm 

Position Summary 

The Medical Assistant – Care Coordination supports providers and the Care Coordination team by managing clinical administrative tasks that promote continuity of patient care. Working from the corporate office, this position serves as a central point of communication between patients, providers, pharmacies, insurance companies, and outside agencies. This role does not provide direct patient care in a clinic setting but plays a vital role in ensuring timely coordination of patient needs and provider support. 

Duties/Responsibilities 

  • Manage and respond to patient portal messages, routing concerns to the appropriate provider when needed.  
  • Assist providers with patient care coordination and follow-up activities.  
  • Coordinate communication with pharmacies regarding prescription refill requests, medication questions, and prior authorizations. 
  • Supports clinic systems and processes that help standardize the patient experience. 
  • Complete and process FMLA, disability, Social Security, and other medical paperwork for patients accurately and within required timelines.  
  • Coordinate with internal departments and external organizations to facilitate patient care.  
  • Maintain accurate documentation within the electronic medical record (EMR). 
  • Maintain patient confidentiality and comply with HIPAA and organizational policies.  
  • Perform other duties as assigned to support patient care and clinic operations.  

 

 

Qualifications 

  • High school diploma or equivalent required.  
  • Current Medical Assistant certification or degree  
  • Minimum of one year of experience in a medical office or care setting preferred.  
  • Experience using an electronic medical record (EMR) system.  
  • Strong organizational, communication, and time management skills.  
  • Ability to manage multiple priorities in a fast-paced environment while maintaining attention to detail.  
  • Excellent customer service and interpersonal skills.  

 

Physical Requirements 

  • Ability to sit, stand, and walk throughout the workday.  
  • Frequent use of computers, telephones, and other office equipment.  
  • Ability to occasionally lift up to 25 pounds.  

 

Work Environment 

  • This is a full-time, onsite position requiring attendance in the office five (5) days per week.  
  • Standard business hours with occasional flexibility based on operational needs.
